You are the key to our continuing effort to provide “Hope and Healing Closer to Home” for all the communities we serve. Your gift to the Community Health Foundation campaign will help us raise $1.3 million toward the cost of the project that will create the Radiation Care Center at Langlade Memorial Hospital. For the first time, the people of Langlade County and the surrounding area will be able to receive state-of-the art and comprehensive cancer care and diagnostic services closer to home without the added burden of travel to a more distant location. We gratefully acknowledge your support which both reinforces the success of today's campaign and opens possibilities for future projects. In accordance with your wishes and donation amount, we will acknowledge your donation in one of several ways.

  1. The Community Health Foundation acknowledges the generosity of campaign donors in the community at large. The Foundation names its donors, without listing amounts, in “thank you” advertisements or postings. The announcement of campaign gifts also encourages others to participate in community efforts by acknowledging generous and widespread support.

  2. The Foundation awards permanent recognition to donors whose campaign gifts are $500 or more. For more detail, please read the Undesignated Giving section that follows.

  3. The Community Health Foundation invites donors to choose a Designated Giving Opportunity, whether it is a room, area, or some other aspect of the project. If you are interested in such a legacy, please turn to the section on Designated Giving that follows.

  4. The Foundation encourages donors to choose an appropriate form of recognition so that we may show our sincere gratitude for campaign contributions. We respect and honor the wishes of those who choose to remain anonymous.

UNDESIGNATED GIVING OPPORTUNITIES

By giving an undesignated campaign gift, you choose not to designate it for any particular area within the Hospital but you may choose to honor or memorialize a special person by means of the gift.

Gifts of $500 or more are acknowledged within our donor recognition system. Donor names are displayed prominently in a central, high-traffic area within the Hospital facility in accordance with the following schedule:

  • Pace-Setting Gifts $ 25,000+
  • Advance Gifts $ 15,000+
  • Major Gifts $ 5,000+
  • Primary Gifts $ 1,000+
  • Community Gifts $ 500+

The recognition system reflects the various giving levels.
